If you are interested in attending camp as a camper, please read the following information carefully:

1. Campers must be no younger than 12.  Campers older than 21 years may apply now - we have removed the upper age restrictions.

2.  To apply as a camper, you must have a disabling condition that affects mobility, or have a need for an accessible camping experience.  Conditions that are likely to affect mobility include (but are not limited to) cerebral palsy, spina bifida, muscular dystrophy, limb amputee, brain injuries, paralysis, etc.  Individuals with other conditions that may require a need for an accessible camping experience, such as severe vision impairment, some genetic conditions, and other health impairments are welcome to apply, if their disability affects their mobility.  Our staff is not able to accomodate individuals with psychiatric, emotional, or severe behavioral disorders.  We are also unable to accomodate individuals who are aggressive or violent, have a history of self abuse or suicidal behavior, have a history of running away, firesetting, or have severe eating disorders.  If you have questions about the appropriateness of a referral, please email the camp administration at info@campsock.org.  The Board of Southern Oregon Camp Kiwanis reserves the right to determine which applicants will be accepted, based on our staff’’s ability to provide appropriate care for campers.

3  Print the camper application.  Complete all pages.  The physician’s statement and standing orders page must be taken or mailed to your doctor.  He or she must fill it out and sign it. (If you have been seen by your doctor at least once in the previous year, you can have him/her fill out the information without going in for an appointment.)  If you have not been seen by your doctor in the previous year, we recommend that you go in for a physical to make sure you are in good health for camp.  We must have this form signed by your doctor in order for you to participate in swimming, horseback riding, and other activities.  Return this form with your camper application, or have your doctor send it to us.
