Physician’s Statement and Standing Orders:  Print this form and take to your doctor to fill out.  (If you have been seen by your doctor at least once in the previous year, you can have him/her fill out the information without going in for an appointment.)  If you have not been seen by your doctor in the previous year, we request that you go in for a physical to make sure you are in good health for camp.  We must have this form signed by your doctor in order for you to participate in swimming, horseback riding, and other activities. 

After your doctor has completed the form, it must be returned to us.  There are two ways to do this:

Preferred:  have the doctor return the form to you, and you send it along with your completed application to attend.  (We prefer to get all the paperwork back at once).

Alternate:  have the doctor return the form directly to us, at the following address:
